For the 2025/26 academic year, CDRs, most PS staff will make use of an online form in MyHR and the 'paper-based' forms below will no longer be used. Although, the online form will be virtually identical to the forms below. A step-by-step guide on how to start, maintain and complete your online form can be found here

For those staff with access issues who cannot use the online form, the forms below remain available on this page, but as agreed with the individual Department or Division, HR must be informed of this offline completion to ensure completion records. 

To help you complete your form, whether online or offline, we've also provided an example form with an example objective and explanations for each section. This is linked below and you'll notice the form also contains links to short videos to further guide you through the process.

*This version of the form is used for roles where standards vary little year on year and should be agreed for use with your line manager. If your role is not like this, you should continue to use the standard form. 

As part of your CDR, especially leading into your end of year review, you may wish to get feedback from colleagues and stakeholders you work with to help undertsand your strengths and areas for development. Most of the CDR process happens offline in conversations between managers and teams members. The online CDR form aims to capture the outcomes of those discussions and serve as a recod of the team member's achievements and your feedback throughout the year.

Following the end-of-year review, managers will need to submit and complete the CDR from, which officially closes the CDR process for the year. Here is a quick reference guide that explains how to manage your team's CDR in MyHR.
