This form is for passing on a student declaration of disability to the Disability and Mental Health Service (DMHS) at LSE.
If you wish to inform DMHS of your own disability you should not complete this form. Instead please see our Speak with an Adviser page and follow the steps relevant to your enrolment status.
Definition of disability
By disability we mean a physical or mental impairment lasting 12 months or more which has a substantial negative impact on a person's abilities to undertake normal daily activities, as per the definition under the Equality Act 2010.

Methods of declaration
The declaration can be made to you in one of the following ways:
Verbal disclosure: the student tells you they have a disability or long-term condition which has an adverse, long-term (i.e. more than 12 months) effect on their ability to carry out day-to-day activities.
Document sharing: the student has presented you with documentation, usually from a medical or psychological professional, that confirms a disability or symptoms which meet the definition above.
Behaviour: where the evidence of a disability is apparent from the student themselves, for example through their behaviour or language.
Indication: the student has completed a form or made an application (eg. for short-term CEAs, ECs, deferral, extension etc.) and indicated that they consider themselves to have a disability or have described symptoms which meet the definition above.
Purpose of a disability declaration referral
The purpose of sharing this information with DMHS is to facilitate the offer of support and adjustments, and meet LSE’s legal and moral obligations to our students.
If a student needs more urgent help relating to disability-related issues then they should also be directed to the appropriate process. For example applying for an extension, Exceptional Circumstances or short-term Central Exam Adjustments.
Please note, if you have concerns about the student’s safety, you should not complete a declaration form, but follow the Cause for Concern process instead.
Upon receipt of this form, DMHS will check whether the student is already receiving support and follow up as appropriate, usually by contacting the student to invite them to meet with a DMHS Adviser to discuss support and adjustments. The student will be contacted within 3 working days. The referrer will receive a response on receipt of the form and further updates as appropriate.
